AIRnyc Patient Story

AS, a 44-year-old resident of Manhattan and patient at Sun River Health, turned to AIRnyc for additional help managing her diabetes and hypertension. Faced with poor A1C levels and other serious health challenges, AS was in need of an extensive support network to help her forge a healthier path.

After a thorough assessment, AIRnyc Community Health Worker (CHW) Michelle Velez - while integrated into Sun River Health’s electronic medical record system - immediately connected AS with a range of specialists, including an endocrinologist, neurologist, and pain management physician. Velez has also attended medical appointments with AS and continuously follows up on her care. “I appreciate all the phone calls from Michelle and having someone listen to me,” AS said.

In addition to facilitating connections to care, Velez has also provided health coaching and social needs referrals. She was instrumental in obtaining a glucometer for AS so she could measure her blood sugar levels. Velez also provided utility bill assistance and connections to mental health services to help ease stress and domestic challenges.

“If I didn’t have the support of my CHW, I wouldn’t feel motivated to take care of my blood sugar and other health issues,” AS pointed out. “CHWs help to remind us and inspire us to actually take care of our health,” she added.

As a result of Velez’s support and interventions, AS’s A1C levels have gone down. She’s also eating more vegetables and eating less carbohydrates which has contributed to her improved blood sugar levels. “Having access to fresh foods is very important. It’s the key to bettering your health,” AS pointed out.

She is grateful to Velez and AIRnyc for inspiring her to make dietary changes and for helping to steer her in the right direction. “A lot of people in the community need extra help,” AS explained. “AIRnyc treats people like people and provides information on available resources that people may not be aware of. CHWs improve the health and wellness of New Yorkers because they truly connect with people.”
