Janise Kangombe

Director of Care Coordination

Janise is a Licensed Social Worker who, supported by her success in managing social service programs and leading innovative teams, strives to be a transformational leader in her field.  

As AIRnyc’s Director of Care Coordination, Janise brings creativity and knowledge of modern evidenced-based practices such as performance management, quality assurance, and partner-relationship management. She advocates incorporating different modalities of technology, utilizes a social-justice and trauma-informed lens in practice, and is extremely passionate about program management in the community. 

A New York native, Janise has served on the front lines as a Direct Care Manager and Community Health Worker (CHW) for varying NYC populations. She has been a key player in helping organizations execute outreach programs and has direct experience with helping to create an in-house CHW certification course that has successfully certified over 25 NYC CHWs remotely.

Previously, Janise was a Supervisor of Community Programs at Asian Community Care Management (ACCM) serving vulnerable New Yorkers through the contracted OneCity Health’s Asthma Home-Based Environmental Program (funded by the New York State Medicaid Waiver Program). She also partnered with Mount Sinai’s Health Home Program. 

Janise also works as a therapist with a focus on transpersonal psychology, is an avid advocate for quantum computers, and serves as a volunteer member of the NYC Department of Health & Mental Hygiene Medical Reserve Corps. She holds a leadership-based Masters of Social Work degree from Fordham University and a Bachelor of Science degree in Social Work from CUNY York College.
